Job Summary

We are seeking an experienced and skilled Heart Failure Specialist Nurse to join our team at South Tyneside and Sunderland NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality patient care and contributing to the development of our services.

Main Responsibilities
  • Provide comprehensive and specialist nursing care to patients with heart failure, including assessment, planning, and evaluation of care pathways.
  • Work collaboratively with the Heart Failure Specialist Nursing team and wider cardiology team to deliver evidence-based therapies and improve patient outcomes.
  • Develop and maintain advanced knowledge and skills within the specialist sphere of practice, staying up-to-date with the latest research and clinical guidelines.
  • Support the development of nursing practice within Cardiology, contributing to the body of nursing knowledge and promoting excellence in patient care.
  • Exercise autonomy and accountability in your practice, adhering to the NMC Code and Trust policies.
  • Contribute to training and education within the specialist area, sharing your expertise with colleagues and supporting the development of others.
  • Provide specialist clinical advice and support to patients and clinical staff, promoting best practice and improving patient outcomes.
  • Represent the service nationally, promoting the work of our Trust and contributing to the development of heart failure services.
Requirements
  • Current 1st level NMC registration.
  • Degree in nursing studies or relevant clinical degree.
  • Mentorship certificate or Practice Assessor training.
  • Clinical skills accreditation or willingness to complete.
  • Leadership and Management Course (desirable).
  • Basic knowledge of financial management (desirable).
  • Non-medical prescribing or willingness to complete (desirable).
Essential Skills and Experience
  • Experience of care delivery and the specialism, normally gained through working at Band 5.
  • Leadership and management experience, including evidence of leading a team and innovation in practice.
  • Experience of coordinating care for a caseload of patients and budget management.
  • Participation in recruitment and selection processes and experience in conducting research and critiquing clinical evidence.
  • Knowledge of current professional and clinical governance issues, research, and clinical audit.
  • Proven interpersonal skills, including the ability to assess, deliver, evaluate, and benchmark quality care.
Physical Skills
  • Ability to assist patients to move as per Trust Moving and Handling Policy.
  • Carrying teaching equipment such as laptop, stationary, etc.
  • Manual dexterity and up-to-date working knowledge of equipment, such as syringe drivers and infusion pumps.
  • Full UK driving licence and access to a car is essential to this role.
