NursLink Homecare is seeking a dedicated and compassionate Registered Nurse to provide essential nursing care to individuals in their homes, residential care settings, or community clinics. The successful candidate will work closely with multidisciplinary teams to promote health, independence, and quality of life.

Key Responsibilities:

* Provide high-quality nursing care to patients in their own homes and community settings, ensuring dignity and respect.
* Assess, plan, implement, and evaluate individualized care plans for patients with complex needs.
* Administer medications, wound care, injections, and other nursing procedures as needed.
* Offer support and guidance to patients and their families, promoting health education and disease prevention.
* Collaborate with GPs, social workers, and other healthcare professionals to ensure holistic patient care.
* Conduct risk assessments and ensure that care is delivered in accordance with NHS guidelines and best practices.
* Support the discharge process by coordinating care for patients transitioning from hospital to home.
* Provide end-of-life care, ensuring patient comfort and dignity.

Requirements:

* Registered Nurse (RN) with a valid NMC Pin.
* Experience in community healthcare or district nursing (preferred but not essential).
* Knowledge of health and safety regulations and infection control practices.
