Primary Care Nurse

About the Role:
We are seeking a highly motivated and enthusiastic Primary Care Nurse to join our forward-thinking team at HMP High Down. As a member of our Prisons Health Care Team, you will play a vital role in delivering patient-focused quality care to individuals residing in secure environments.
Main Responsibilities:
- Triage patient needs in accordance with specific standard operating procedures and national policy e.g. NICE guidance
- Assess, plan, and implement care for patients within each care/treatment pathway to promote wellbeing
- Facilitate evidence-based practice to ensure effective and appropriate clinical care
- Carry out general nursing duties, including maintaining contemporaneous records
- Support and contribute to the development of integrated healthcare services
Requirements:
- Registered (NMC) Adult Nurse
- Numeracy skills for safe medication administration
- Previous experience in Primary Care services or equivalent
Salary: £40,000-£55,000 per annum, depending on experience
