About the Role

We are seeking a highly motivated and compassionate Dementia Adviser to join our team in North Lincolnshire. As a Dementia Adviser, you will play a vital role in providing support, information, and guidance to people living with dementia and their families.

Key Responsibilities

  • Provide person-centered support to individuals living with dementia, promoting independence and well-being.
  • Work closely with healthcare professionals, social services, and other stakeholders to ensure seamless care coordination.
  • Develop and maintain relationships with local communities, promoting awareness and understanding of dementia.
  • Conduct assessments and develop personalized care plans to meet the unique needs of each individual.
  • Provide education and training to family members and caregivers on dementia care and support.

About You

We are looking for a dedicated and empathetic individual with a passion for supporting people living with dementia. You will have excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.

Requirements

  • Experience working with people living with dementia or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Basic IT skills and proficiency in using databases and virtual meeting software.
