About the Role

We are seeking a highly motivated and compassionate individual to join our team as a Dementia Adviser. As a key member of our community outreach team, you will play a vital role in providing support, information, and guidance to individuals and families affected by dementia.

Key Responsibilities
  • Deliver dementia awareness and education sessions to community groups and individuals
  • Provide one-to-one support and guidance to people living with dementia and their carers
  • Work closely with local health and social care professionals to ensure seamless service delivery
  • Develop and maintain relationships with community partners to promote our services
  • Contribute to the development of new initiatives and projects to improve dementia support in the community
About You

We are looking for someone who is passionate about making a difference in the lives of people affected by dementia. You will have excellent communication and interpersonal skills, with the ability to work independently and as part of a team. A strong understanding of dementia and its impact on individuals and families is essential, as is the ability to work in a fast-paced environment.

Experience of working in a community outreach role or a similar setting is desirable, as is knowledge of local health and social care services. You will be required to travel across North Lincolnshire, so a full driving license and access to a vehicle is essential.
