Job Overview

We are seeking an experienced General Practice Nurse to join our dynamic team in a GP practice in Wales. This is an exciting opportunity for a motivated and dedicated professional to work in a supportive environment, providing high-quality patient care.

Key Responsibilities
  • To provide and maintain a high standard of nursing care to patients, with a focus on evidence-based practice for long-term conditions and acute problems.
  • Clinically examine and assess patient needs, planning care accordingly.
  • Assess, diagnose, plan, implement, and evaluate interventions/treatments for patients with complex needs.
  • Proactively identify, diagnose, and manage treatment plans for patients at risk of developing long-term conditions.
  • Diagnose and manage both acute and chronic conditions, integrating drug- and non-drug-based treatment methods into a management plan.
Requirements
  • Registered Practice Nurse with experience working in Primary Care.
  • Knowledge of health promotion strategies and experience of chronic disease management, vaccinations, and immunisations.
  • Experience working within a nursing team.
Benefits

We offer a competitive salary range £28,000 - £36,000 per annum, depending on experience, along with opportunities for professional development and growth.
