About the RoleWe are seeking an experienced and compassionate Adult Social Worker to join our dynamic team at Ben Recruitment Ltd. As an Adult Social Worker, you will play a crucial role in assessing and supporting the needs of adult patients, ensuring they receive the necessary care and resources to improve their overall well-being.
Key Responsibilities- Conduct comprehensive assessments of adult patients to identify their social, emotional, and psychological needs.
- Develop personalized care plans for patients, in collaboration with other healthcare professionals, to address their specific needs and concerns.
- Provide counseling and support to patients and their families, helping them cope with the challenges and difficulties they may face during their healthcare journey.
- Coordinate with community organizations and resources to ensure patients receive appropriate community-based services and support after discharge.
- Collaborate with interdisciplinary teams, including doctors, nurses, and therapists, to provide holistic care to patients.
- Advocate for patients' rights and access to necessary services, ensuring their voices are heard and their needs are met.
- Monitor and evaluate the effectiveness of interventions and adjust treatment plans as necessary to promote positive outcomes.
- Bachelor's or master's degree in Social Work or a related field.
- State licensure as a Social Worker is required.
- Minimum of 4 years of experience working as an Adult Social Worker in a healthcare setting.
- Strong knowledge of social work principles, theories, and techniques.
- Excellent communication and interpersonal skills, with the ability to establish rapport with patients and their families.
- Empathy and compassion for patients going through challenging situations.
- Proficient in using electronic medical records and other healthcare software systems.
- Ability to work effectively in a fast-paced and high-pressure environment.
- Flexible and adaptable approach to work, with the ability to prioritize and manage multiple tasks simultaneously.
- Strong problem-solving and critical-thinking skills, with the ability to make sound judgments in complex situations.
