Job Summary:
We are seeking a highly skilled and experienced Community Psychiatrist Nurse to join our Dementia and Older People's Mental Health Service in Cornwall Partnership NHS Foundation Trust.
Main Responsibilities:
- Provide assessment, treatment, and care coordination to individuals living with dementia and those with functional mental health needs associated with physical ill health and the physiology of ageing.
- Work closely with a wide range of statutory and non-statutory agencies to develop and deliver personalized care under the care program approach.
Requirements:
- Current professional registration with the NMC
- Postgraduate qualification or equivalent experience in a psychological therapy, such as CBT
- Substantial experience of managing a busy caseload of clients with dementia and mental health problems
- Providing clinical supervision
Working Environment:
The post is based at Longreach House, Redruth, and involves visiting people at home, in care homes, and supporting community health and social care teams.
Person Specification:
- A working knowledge of the Mental Health Act, the Mental Capacity Act, and Deprivation of Liberty Safeguards
