We are seeking a highly skilled and experienced Community Psychiatrist Nurse to join our team at Cornwall Partnership NHS Foundation Trust. As a key member of our multidisciplinary team, you will provide assessment, treatment, and care coordination to individuals living with dementia and older adults with complex mental health needs.
Key Responsibilities- Provide comprehensive assessments and develop personalized care plans for clients with dementia and mental health disorders.
- Work closely with carers, family members, and community health and social care teams to ensure seamless care delivery.
- Manage a caseload of clients under the Care Programme Approach, prioritizing their needs and ensuring timely interventions.
- Collaborate with a wide range of statutory and non-statutory agencies to develop and deliver personalized care under the care program approach.
- Professional qualification in nursing (RMN) and current registration with the NMC.
- Postgraduate qualification or equivalent experience in a psychological therapy, such as CBT.
- Substantial experience of managing a busy caseload of clients with dementia and mental health problems.
- Proven ability to work in a multi-disciplinary and multi-agency environment.
- Computer literacy and working knowledge of relevant legislation, including the Mental Health Act and the Mental Capacity Act.
