The successful candidate will have relevant professional registration with the NMC or HCPC and experience of working with people with learning disabilities or complex health needs.

Key Responsibilities

  • Assess service users' needs, devise care plans, and evaluate outcomes that promote empowerment.
  • Provide high-quality, individualized care that addresses the needs of service users and meets individual health needs.
  • Develop and review comprehensive and evidence-based clinical risk assessments and associated care plans.

What We Offer

The successful candidate can expect a competitive salary range of £50,000 - £70,000 per annum, depending on experience. You will also benefit from ongoing training and development opportunities, as well as a supportive and collaborative work environment.
