Job Description: Child Care Team Leader

We are seeking a highly motivated and experienced Registered Children's Nurse to join our team as a Child Care Team Leader. This is an exciting opportunity for an ambitious and skilled nurse to take on a leadership role in a well-established provider of complex care packages within the community.

The successful candidate will be responsible for leading a team of carers, training and supporting them with their clinical skills and activities. They will also provide face-to-face support to children and their families, while creating personalised care plans tailored to each child's unique needs.

The ideal candidate will possess up-to-date experience of tracheostomy care and ventilators within the last 2 years and have strong communication skills verbally and written to liaise effectively with patients, families, and multidisciplinary teams. They should also be suitable for Band 5 / Band 6 Nurses with evidence of effective leadership and management of Healthcare Assistants.

Key Responsibilities:
  • Lead a team of carers and provide training and support
  • Design and deliver a tailored care plan specific to an individual client's needs
  • Effectively manage the multidisciplinary team through strong communication and teamwork
  • Provide ongoing monitoring, support, and assessment of clients' physical and physiological needs within your caseload
  • Deliver nursing interventions including the management of medicine delivery
  • Build and maintain sophisticated relationships with your clients' families
  • Work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care
