Job Summary

Main Duties and Responsibilities
  • Provide clinical pharmacy services to patients in the outpatient setting, including patient counselling and medication management.

Person Specification
  • Demonstrable ability to meet the Trust Values.
  • Masters degree or equivalent in pharmacy.
  • Registration with the General Pharmaceutical Council.
  • Mandatory CPD to maintain fitness to practice.
  • Experience of patient counselling and completed audit to improve practice.
  • Desirable: experience of working in a hospital environment, post-registration hospital experience, ward pharmacy experience, and understanding of medicines reconciliation.
