About the Role

We are seeking a highly skilled and motivated Care Coordinator to join our team at Greenwich PCN Alliance Limited. As a Care Coordinator, you will play a vital role in supporting individuals with complex care needs, working closely with healthcare professionals, families, and carers to develop and implement personalized care plans.

Main Responsibilities
  1. Work collaboratively with healthcare professionals, families, and carers to identify and address the care needs of individuals, developing and implementing personalized care plans to promote independence and improve quality of life.
  2. Conduct thorough assessments of individuals' care needs, identifying areas for improvement and developing strategies to address these needs.
  3. Work closely with healthcare professionals to ensure seamless care coordination, facilitating communication and collaboration to achieve optimal outcomes.
  4. Provide education and support to individuals, families, and carers on care planning, self-management, and health promotion.
  5. Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.
Requirements
  • Proficient in MS Office and web-based services.
  • Demonstrable commitment to professional and personal development, with a focus on personalized care and support planning.
  • NVQ Level 3 in adult care - advanced level or equivalent qualifications or working towards.
  • Experience of working within multiprofessional team environments, with a focus on care coordination and support planning.
  • Excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and carers from diverse backgrounds.
