Care Coordinator Mental Health Nursing/Social Worker/OT Specialist

We are seeking a highly motivated and experienced Care Coordinator to join our Ealing Community Rehabilitation Team. As a key member of our multidisciplinary team, you will play a vital role in providing support to individuals with complex care needs related to psychosis and co-existing substance use and physical health problems.
Main Responsibilities- Assess, plan, implement, and evaluate comprehensive care packages for clients with severe and enduring mental health problems.
- Develop and deliver psychosocial interventions to support clients and their families.
- Work collaboratively with multidisciplinary teams to provide intensive support and rehabilitation in the community.
- Contribute to the development and implementation of individualized care plans.
- Provide education and training to clients, carers, and other professionals.
- Current professional qualification in Mental Health Nursing/Social Work/OT.
- Registration with a professional body such as NMC or other relevant body.
- Evidence of post-registration training and CPD.
- Experience of working in multi-disciplinary mental health teams with clients with severe and enduring mental health problems.
- Knowledge of CPA process and current legislation and guidance applicable to this client group.
