As a key member of our multidisciplinary team, you will play a vital role in providing support to individuals with complex care needs related to psychosis and co-existing substance use and physical health problems.
Key Responsibilities- Develop and implement comprehensive care plans for service users, working closely with multidisciplinary teams to ensure seamless delivery of care.
- Conduct thorough assessments and risk assessments to identify individual needs and develop tailored support plans.
- Provide expert guidance and support to service users, carers, and other professionals to promote recovery-focused care.
- Collaborate with colleagues to develop and deliver training programs, ensuring staff have the necessary skills and knowledge to provide high-quality care.
- Contribute to the development and implementation of policies and procedures, ensuring they are aligned with best practice and regulatory requirements.
- Current professional qualification in Mental Health Nursing, Social Work, or Occupational Therapy.
- Registration with a professional body such as the NMC or other relevant organization.
- Evidence of post-registration training and continuing professional development.
- Experience working in multidisciplinary mental health teams with clients with severe and enduring mental health problems.
- Knowledge of the Care Programme Approach (CPA) process.
- Excellent verbal and written communication skills, including report writing and presentations.
- Ability to assess, plan, implement, and evaluate comprehensive care packages for identified client groups.
