This role involves delivering high-quality nursing care to patients in their own homes or community settings, supporting them to maintain independence and achieve their healthcare goals.

The ideal candidate will have experience in working autonomously, managing a daily caseload, and prioritizing tasks to ensure safe and effective care delivery. They will also be required to work collaboratively with other healthcare professionals, including GPs, therapists, and social workers, to ensure coordinated care.

We offer a competitive salary of £38,000 - £48,000 per annum, depending on experience, and a range of benefits, including flexible working hours, access to continuing professional development (CPD) pathways, and a supportive and inclusive working environment.

Job Description:

Deliver high-quality nursing care to patients in their own homes or community settings, supporting them to maintain independence and achieve their healthcare goals.

Work autonomously in patients homes and community settings, ensuring safe and effective care delivery.

Manage a daily caseload, prioritising tasks and responding to changes in patients conditions.

Required Skills and Qualifications:

  • Registered Nurse (Adult) with current NMC registration.
  • Clinical experience in a relevant setting, such as community, primary care, or acute services.
  • Ability to work independently and manage a caseload in a community environment.
  • Strong clinical assessment and decision-making skills.
  • Excellent communication, interpersonal, and organisational skills.
  • A full UK driving licence and access to a vehicle for work purposes (if applicable).
  • A commitment to patient-centred care and promoting independence.
