Young Adult Care Coordinator

We are seeking an experienced and compassionate individual to coordinate care and support services for young adults with complex needs.
About the Role:As a Young Adult Care Coordinator, you will oversee the delivery of personalized care plans, ensuring seamless coordination between care teams and healthcare professionals.
Key Responsibilities:- Developing and implementing care plans tailored to individual needs
- Collaborating with healthcare professionals to ensure comprehensive care
- Coordinating care services, including medical procedures and therapies
- Providing support and guidance to care teams
- Experience working in a care setting – preferably with young adults
- Ability to work on a rota pattern, including every other weekend
- Excellent communication and organizational skills
- Proven ability to coordinate multiple stakeholders
- A competitive hourly rate of £11.78 (linked to competency and qualifications)
- A welcome bonus of £1500
- Full training and continued support
- Opportunity to study for Level 3 diploma in Health and Social Care (funded by us)
- Part-time hours available if required
