Care Coordinator Job Description

We are seeking a skilled Care Coordinator to join our Community Team for People with Learning Disabilities in the Royal Borough of Windsor and Maidenhead.

Job Purpose

The successful candidate will work closely with adults with learning disabilities and their families and carers to assess their needs and develop care plans to support them in leading fulfilling lives.

You will be responsible for carrying out assessments, undertaking care reviews and providing support to enable adults with learning disabilities to access the services they need.

Key Duties and Responsibilities
  • Assess the needs of adults with learning disabilities and develop care plans to support them
  • Collaborate with other professionals to provide support to adults with learning disabilities
  • Participate in a duty rota to respond to incidents and safeguarding alerts
  • Contribute to the development and implementation of policies and procedures related to safeguarding adults with learning disabilities

Salary: £37,080 to £40,788 per annum
