Clinical Lead Evening and Nights Community Nursing - Development Post
This is an exciting opportunity to develop your career and make an impact within the out of hours community nursing team and the patients we care for in the evening and night.
You will be somebody who is adaptable and flexible to meet the needs of the service, looking to develop in their career, with or without Community Nursing experience that has transferable skills.
You will be somebody who is willing to develop in advanced leadership skills alongside their clinical skills.
This role includes enhanced pay at time and a third after 8 pm and many other benefits.
Main Duties of the Job
- To provide direction within the caseload in consideration of the impact of current health and social care agendas and local initiatives
- To focus on the delivery of person-centred care
- To provide effective, evidence-based care to a defined population through caseload management and monitoring of care standards
- Provide leadership including clinical supervision to team members and provide support to the Out of Hours Community Team managerial lead

Qualifications
Essential
- First level registered general adult nurse
- Evidence of teaching and assessing
- SPC qualification in District Nursing or equivalent; demonstrating robust evidence of caseload management, community profiling and application of health needs assessment
Knowledge and Expertise
Essential
- Proficiency in delivering high quality evidenced based care
- To be able to demonstrate continuing professional development
- Ability to work and communicate effectively within a MDT setting
Experience
Essential
- Significant experience of working within a community healthcare setting
- Experience of making autonomous decisions
