About the Role

We are seeking a dedicated and compassionate Registered Mental Health Nurse, Occupational Therapist, or Social Worker to join our team as an Adult Mental Health Practitioner. This exciting opportunity will allow you to work with older adults, providing high-quality care and support in a dynamic and supportive environment.

Key Responsibilities
  • Conduct memory assessments and semi-structured interviews with patients and their families to identify care needs and develop personalized care plans.
  • Collaborate with the integrated Older Adults Mental Health team to provide advice and support to patients and their carers.
  • Assess and manage risk in various settings, incorporating principles of Clinical Governance.
  • Work closely with patients and their carers to ensure their involvement in the assessment and care planning process.
