About the Role

We are seeking a highly skilled and compassionate Adult Mental Health Practitioner to join our Older Adults Community Mental Health Team. As a key member of our team, you will provide high-quality mental health care and support to older adults in our community.

Key Responsibilities
  • Conduct thorough assessments of patients, ensuring active involvement of their caregivers and family members in developing comprehensive care plans.
  • Take ownership of formulating, implementing, and assessing the effectiveness of care plans for a specific group of patients, including those with complex needs, in close collaboration with patients and the multi-disciplinary team.
  • Offer direct supervision and guidance to Support Workers and/or students, fostering their growth and development within the team.
Requirements
  • Registered Mental Health Nurse or equivalent.
  • Good understanding of the relevant legislation surrounding the Mental Capacity Act / Mental Health Act.
  • Ability to drive and have a car for use at work.
