Community Care Coordinator Role

As a Community Care Coordinator, you will play a vital role in coordinating visits and managing the administration of holistic care in the community. You will work closely with colleagues to ensure that patients receive timely, evidence-based, and compassionate care.
**Key Duties:**
- Contributing to processes for proactively case finding patients who are very high intensity users of community and primary healthcare and/or are at high risk of unplanned admission to hospital.
- Assessing the physical and psycho-social needs of patients and developing tailored interventions to meet their individual needs.
- Improving health outcomes using specialist nursing skills and knowledge.
- Managing complex cases and developing effective case management programs.
**Person Specification:**
- A registered Allied Healthcare Professional or Registered Level 1 Nurse with a Degree/Diploma in Health/Nursing Studies or related subject.
- Evidence of recent Continuous Professional Development and relevant postgraduate clinical training.
- Excellent communication and leadership skills, with the ability to motivate staff and develop partnership working.
