Job Title: Advanced Nurse Practitioner - Lead Role

We are seeking an experienced and skilled Advanced Nurse Practitioner to join our team as a Lead Nurse for the Physical Disability Continuing Healthcare (CHC) Team.

About the Role

The successful candidate will be responsible for undertaking CHC assessments and reviews for patients across all aspects of care, requiring a high level of assessment skills and the ability to liaise effectively with colleagues, healthcare professionals, service users, carers, and family members.

Key responsibilities include developing and maintaining effective communication networks with health and social care professionals, working with relevant health professionals to ensure a seamless and efficient service delivery, and providing specialist nursing advice to secondary care, primary care, and other healthcare settings.

Person Specification
  • Registered Nurse (RGN/RMN/RNLD/RSCN) with current and relevant NMC registration
  • Educated to degree level or equivalent in experience
  • Post Graduate qualification or specialist training in Community Nursing
  • Minimum 2 years post-qualification experience as a Registered Nurse
  • Evidence of on-going professional development
  • Significant experience of both community and hospital working and a sound understanding of the NHS CHC national guidance
  • Demonstrable evidence of the use of key performance indicators
  • Extensive knowledge of the Mental Capacity Act and Deprivation of Liberty Safeguards
  • Up-to-date knowledge of the national nursing agenda
