Job Title: Advanced Community Practitioner

Slaithwaite Health Centre is seeking an experienced Advanced Community Practitioner to join their team. The successful candidate will be responsible for managing patients with acute and chronic medical conditions, with a focus on frailty patients.

Main Responsibilities:
  • Clinically examine and assess patient needs, planning care accordingly in the practice and during home visits.
  • Oversee the Frailty Register and assess, diagnose, plan, implement, and evaluate interventions/treatments for patients with complex needs.
  • Prioritize health problems and intervene appropriately in complex, urgent, or emergency situations, including initiation of effective emergency care.
  • Refer patients to other healthcare professionals as necessary, in accordance with local framework/guidelines.
  • Meet the needs of patients presenting for opportunistic long-term condition management or assessment.
  • Support advanced care planning for end-of-life patients.
  • Undertake general telephone triage and first-contact assessment.

Person Specification:
  • Advanced Clinical Practitioner Masters Degree
  • Experience of working in the community with frailty patients
Requirements:
  • Experience of working in a primary care environment
  • Experience of chronic disease management
  • Experience of working autonomously
  • Experience of infection prevention and control measures
  • Experience of managing elderly and housebound patients
