Working with Us:

As a Primary Care Nurse Specialist at Southwell Medical Centre, you will be part of a dedicated team committed to delivering high-quality patient care. You will work closely with local services and agencies to ensure seamless patient care and contribute to the development of innovative health promotion and wellbeing programs.

Key Responsibilities:
  • Develop, implement, and embed health promotion and wellbeing programs tailored to community needs.
  • Recognize, assess, and refer/signpost patients presenting with mental health needs where clinically indicated.
  • Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice.
Requirements:
  • Registered with the Nursing and Midwifery Council.
  • Masters degree qualification in line with RCN Credentialing for Advanced Level of Nursing Practice.
  • Experience of practice within the Four Pillars.
  • Demonstrable experience of advanced nursing practice.
Salary:

An estimated salary of £43,404 per annum, based on a 37-hour working week, is available for this position.
