Advanced Healthcare Practitioner

Southwell Medical Centre is a semi-rural practice serving a patient population of circa 12,300 individuals. We pride ourselves on our collaborative approach to healthcare, working closely with local services and agencies to ensure seamless patient care.
Job Description:This role involves working as part of a dynamic team to deliver comprehensive primary care services. You will be responsible for developing, implementing, and evaluating health promotion and wellbeing programs tailored to the specific needs of our community. This may include managing acute and chronic conditions, identifying and supporting at-risk patients, and recognizing and assessing mental health concerns.
Skills and Qualifications:- Master's degree qualification in line with RCN Credentialing for Advanced Level of Nursing Practice.
- Experience of prescribing and undertaking medication reviews.
- Excellent communication and interpersonal skills.
- Ability to work effectively as part of a team.
We offer a competitive salary of £43,404 per annum, along with access to the NHS Pension Scheme, on-site parking, and an employee assistance program.
