Job Title: Complex Mental Health Support Worker

We are seeking a skilled and compassionate Complex Mental Health Support Worker to join our team at CJP Outreach Services.

The ideal candidate will have a background in supporting individuals with complex mental health needs, including personality disorders, complex post-traumatic stress disorder, and autism.

Responsibilities will include:

  • Providing trauma-informed, person-centred support to individuals with complex mental health needs.
  • Developing and implementing tailored support plans to promote independence and self-management.
  • Collaborating with individuals, families, and external professionals to ensure holistic care.
  • Implementing Positive Behaviour Support (PBS) strategies to improve quality of life and reduce behaviours of concern.
  • Assisting with medication management, medical appointments, and maintaining good health and well-being.
  • Supporting individuals to engage in community activities, access local services, and build meaningful relationships.
  • Maintaining accurate and comprehensive care records.

Hours:

This role requires a 24/7 staffing ratio of 2:1 support, with rotational shifts including every other weekend and sleep-in duties.
