Join Central and North West London NHS Foundation Trust as a Clinical Care Specialist and play a vital role in providing high-quality healthcare services to young people and young adults in secure environments.

We are seeking highly skilled and compassionate Band 5 Nurses to join our team at HMP YOI Feltham. As a Clinical Care Specialist, you will work closely with multidisciplinary teams to deliver patient-focused care, promoting a trauma-informed environment and empowering young people to understand their own health and wellbeing needs.

You will assess, plan and implement care for patients within each care/treatment pathway, utilising evidence-based practice to ensure clinical care is both effective and appropriate to the individual's needs.

Main Responsibilities:

  • To carry out clinical duties such as assessing, planning, implementing and evaluating care for patients using agreed Patient Group Directives or care pathways where appropriate.
  • Administer medication as prescribed, including oral, immunisations, intramuscular injections, insulin or nebulisers, taking into account the complexity of the young person's needs.
  • Conduct treatments where possible (e.g. Hep B and Flu vaccines, minor ailments), referring on to other departments, clinical specialists or any other appropriate person as required.
  • Refer young people identified as requiring specialist input (such as mental health or substance misuse) to relevant services in a timely manner and ensure relevant paperwork is completed.
  • Complete ACCT documents for young people at risk of self-harm, communicating concerns to relevant staff members.
  • Deliver health promotion interventions to young people and advise staff accordingly, promoting positive health and wellbeing outcomes.
  • Participate in health promotion/harm minimisation clinics in specific disease-related areas, ensuring smooth clinic operations and adherence to national and local priorities and guidelines.
  • Collect or transport specimens as required (e.g. blood, wound swabs or urine).
  • Ensure young people leaving care have sufficient medication and a discharge letter outlining their care package for future reference.
  • Perform CPR as necessary and utilise emergency equipment effectively.
  • Attend case conferences as required for patients with challenging behaviour or those on ACCT.
  • Pursue additional tasks as reasonably required, aligning with post responsibilities and grade.

Compensation Package: £32,000 per annum, plus benefits
