Job Title: Community Health Specialist

Estimated Salary: £35-40 per hour
The role of a Community Health Specialist is to provide health guidance and support to families with young children in the Barking area. As a key member of our team, you will work closely with parents and caregivers to promote healthy habits and prevent illnesses.
About the Role:
- Conduct home visits to assess the health and developmental needs of children and families
- Develop and implement individual care plans tailored to meet the specific needs of families
- Provide education on health promotion, parenting skills, and child development
- Collaborate with other healthcare professionals to ensure comprehensive care for families
- Maintain accurate records of assessments, interventions, and outcomes
- Lead community health initiatives to raise awareness about available services and resources
Requirements:
- Registered Nurse qualification with experience in community health or paediatrics preferred
- Strong leadership skills to guide families through health challenges
- Excellent communication skills to effectively engage with diverse populations
- Proven time management abilities to manage multiple cases efficiently
- Familiarity with developing care plans that address individual family needs
