The role of a Community Health Specialist is to provide health guidance and support to families with young children in the Barking area. As a key member of our team, you will work closely with parents and caregivers to promote healthy habits and prevent illnesses.

About the Role:

  • Conduct home visits to assess the health and developmental needs of children and families
  • Develop and implement individual care plans tailored to meet the specific needs of families

  • Collaborate with other healthcare professionals to ensure comprehensive care for families

Requirements:

  • Registered Nurse qualification with experience in community health or paediatrics preferred
  • Strong leadership skills to guide families through health challenges
  • Excellent communication skills to effectively engage with diverse populations
  • Proven time management abilities to manage multiple cases efficiently
  • Familiarity with developing care plans that address individual family needs
