Job Title: Social Worker for Children with Disabilities Team

We are seeking a skilled Social Worker to join our Children with Disabilities Team, working with children and young people who have complex needs, including physical and developmental disabilities.

Your key responsibilities will include:

  • Assessing the needs of children and young people, and creating individualised care plans to support their independence, well-being, and quality of life.
  • Facilitating access to services that enhance the lives of children and young people with disabilities.
  • Working collaboratively with families, carers, and other professionals to ensure the best possible outcomes for children and young people.

We are looking for a dedicated and compassionate Social Worker who is passionate about making a difference in the lives of children and young people with disabilities.
