We are seeking a highly skilled and experienced Senior Practitioner to join our Strengthening Independence Services Disabled Young People's Team in East Kent. As a Senior Practitioner, you will play a key role in supporting disabled young people aged 18-25 with a range of care and support needs.

About the Role:

As a Senior Practitioner, you will be responsible for assessing the needs of disabled young people and developing SMART plans to support their skills and independence. You will also undertake Mental Capacity Assessments, Court of Protection applications, and Section 42 safeguarding enquiries.

Key Responsibilities:
  • Assess the needs of disabled young people and develop SMART plans to support their skills and independence
  • Undertake Mental Capacity Assessments, Court of Protection applications, and Section 42 safeguarding enquiries
  • Supervise, motivate, and mentor a small group of Social Workers and Social Work Assistants
  • Assist the Team Manager to facilitate monthly reflective group supervision, bi-weekly team meetings, and weekly performance meetings
  • Oversee the work of the team in the Team Manager's absence
Requirements:

To be successful in this role, you will need to be a qualified Social Worker with substantial and varied experience of working with either children, young people, or adults. You will also need to be registered with Social Work England and possess a positive, forward-thinking attitude.
