We are seeking a highly skilled and experienced Mental Health Nurse to join our team as a Team Leader. The successful candidate will be responsible for the management of our mental health services, ensuring that high-quality care is delivered to our patients.

Livewell Southwest is an independent social enterprise providing integrated health and social care services. Our teams work across various locations, including community hospitals, GP practices, sports centers, and health and wellbeing hubs.

Job Description
  • Act as care coordinator or named nurse for identified clients following the Electronic Care Programme Approach (eCPA) procedures.
  • Deliver high-standard individualized care, including assessment, planning, implementation, review, and evaluation of care using the eCPA process.
  • Ensure comprehensive risk assessment, history, planning, and management are recorded and communicated to the team and wider service if necessary.
  • Contribute to and be an effective member of the Multidisciplinary team, attending meetings, assessments, reviews, clinical meetings, and case conferences.
  • Work in partnership with carers, offering support, advice, and reassurance on sensitive issues, involving them in care planning where possible.
  • Direct and support junior staff in delivering high standards of individually planned care, creating a learning environment for their development.
  • Maintain accurate clinical notes, CPA paperwork, and other relevant documents in line with NMC guidelines and Livewell Southwest policies.
Required Skills and Qualifications
  • RMN/Dip HE or BSc Mental Health qualification.
  • NMC Registration.
  • Multi-disciplinary working experience.
  • Relevant transferable healthcare experience of working with adults with mental health issues.

Location

The role is based in Plymouth, South Hams, and West Devon areas, with some travel required between locations.
