About the Role

We are seeking a highly skilled and experienced Healthcare Specialist Lead to join our team at Cornwall Partnership NHS Foundation Trust. This is an exciting opportunity to work in a dynamic and supportive environment, contributing to the delivery of high-quality patient care.

The successful candidate will be responsible for managing a clinical caseload within a community nursing team, working closely with patients, carers, and healthcare professionals to provide personalized support plans and promote informed choice.

This role requires strong leadership skills, conflict resolution abilities, and advanced computer skills, including proficiency in Microsoft Office and spreadsheets.

As a key member of our team, you will be expected to collaborate with multidisciplinary teams, promoting safe, effective admission avoidance and supporting timely discharge from hospitals and other care settings.

Key Responsibilities:

  • Manage a clinical caseload within a community nursing team
  • Work collaboratively with patients, carers, and healthcare professionals
  • Develop and implement personalized support plans
  • Promote informed choice and independence
  • Collaborate with multidisciplinary teams to achieve safe, effective admission avoidance and timely discharge

Requirements:

  • Current professional registration - RN (Adult)
  • Evidence of recent relevant professional development
  • A degree in nursing
  • Leadership skills and conflict resolution abilities
  • Advanced computer skills, including proficiency in Microsoft Office and spreadsheets

Estimated Salary: £32,000 - £40,000 per annum
