About the Role

We are seeking a highly skilled and compassionate Nurse Case Manager to join our team at Routes Healthcare (North). As a key member of our clinical team, you will play a pivotal role in supporting and overseeing our complex care packages, ensuring the highest standards of quality and clinical excellence.

Key Responsibilities:

  • Management of complex care packages, both clinically and holistically
  • Developing, reviewing and implementing care plans and risk assessments
  • Manage the clinical training needs of the care team, maintaining an appropriate skill mix to ensure safe and effective clinical practice
  • Engaging in networking, attending MDT meetings and working with clinical commissioning groups
  • New and ongoing competency assessments with the care staff team
  • Care package review
  • Auditing of client notes and medication records

Requirements:

  • RN Qualification (Adults)
  • A valid NMC pin
  • Experience in a high dependency or critical care setting would be advantageous
  • Experience in tracheostomy and ventilation management in a community setting is desirable
  • Be computer literate and have experience of using word, excel and outlook
  • Someone passionate about putting people first
  • Flexible and Autonomous
  • Organisational & time management skills
  • An understanding of CQC regulations
  • Excellent communication skills, both written and verbal
  • Access to a car for work
