Job Summary:
We are seeking an experienced and skilled Band 6/7 Nurse to join our Community-Based Neuro Rehab Team at St Marks Hospital. As a key member of our team, you will deliver high-quality nursing care to patients who have been discharged from hospital following a stroke.
Main Responsibilities:
- Work closely with other Nurses to complete holistic nursing assessments and plan, deliver, and evaluate evidence-based interventions to achieve the best outcome for patients.
- Develop knowledge and clinical competence within stroke and rehabilitation and utilize this to deliver excellent clinical care with the support of Senior Nurses and the Multi-disciplinary Team.
- Use excellent communication skills to engage and motivate Stroke Survivors to engage in rehabilitation and adhere to secondary Stroke prevention advice, as well as liaise with relevant professionals within the team and in wider services.
- See patients in their own homes and work independently following appropriate induction.
- Complete relevant administrative duties such as accurate and concise documentation of notes, reports, and data recording as required by the role.
- Play a role in coordinating referrals, establishing suitability for the service, and attending hospital multidisciplinary meetings as required.
Requirements:
- NMC Registered Nurse with excellent communication skills and a strong interest and experience in stroke and rehabilitation.
- Ability to work closely with Occupational Therapists, Physiotherapists, Speech and Language Therapists, Psychologists, and Therapy Assistants.
What We Offer:
- A competitive hourly rate of £27ph Weekdays.
- A £250 bonus for starting this contract.
- A £250 referral fee for successful referrals.
