Senior Rehabilitation Staff Nurse

We are seeking an experienced and compassionate Senior Rehabilitation Staff Nurse to join our team at Thames Hospice. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to patients with advanced life-limiting illnesses.
Key Responsibilities- Lead the development and delivery of the IPS service, ensuring excellent standards of care for patients and their families.
- Provide day-to-day management of the IPS under the direction of the Sister or Senior Sister.
- Assess and develop individual care plans, using referral criteria, regular patient review, assessment, re-assessment, and discharge criteria as appropriate.
- Deliver direct care to patients, including symptom control and administration of medicines in line with Thames Hospice Policy.
- Communicate effectively with patients, families, and professional colleagues to ensure a safe and needs-led care pathway.
- Work collaboratively with the multidisciplinary team to ensure a coordinated and defined management plan for each patient.
- Maintain accurate healthcare records, liaising with community colleagues and adhering to Thames Hospice organisational policy and professional requirements.
- Participate in training and mentoring of student nurses, external stakeholders, and new staff members.
- Monitor practice and competence for Registered Nurses and Health Care Assistants.
- Contribute to performance management, including recruitment, appraisal, and monitoring staff performance.
- Communicate with external care agencies, GPs, and community teams to facilitate best working practice and service provision.
- Participate in audits to identify improvements and changes that enhance patient and care experiences.
- Report accidents, incidents, and complaints in accordance with procedure.
- Experience as a Registered Nurse, preferably in a hospice or palliative care setting.
- Excellent communication and interpersonal skills.
- Strong care values and a commitment to patient-centered care.
- Ability to work effectively in a team environment.
- Knowledge of palliative care principles and practices.
- Competent in extended skills, including IV drug administration, cannulation, and male catheterisation.
- Awareness of national Care Quality Commission monitoring requirements.
- Ability to maintain accurate records and communicate effectively with patients, families, and colleagues.
- Commitment to ongoing professional development and adherence to Thames Hospice Values.
