Job Title: Care Manager

We are seeking a highly skilled Care Manager to oversee and coordinate patient care services, focusing on individuals with complex care needs, including mental health conditions, personality disorders, Autism, and learning disabilities.

Key Responsibilities:

  • Manage one service, ensuring compliance with healthcare regulations.
  • Lead projects and mentor other managers.
  • Recruit, train, and supervise staff members.
  • Ensure adherence to the Care Act 2014 and CQC standards.
  • Promote a respectful and inclusive environment, empowering staff to support the health and well-being of individuals.

Requirements:

  • NVQ Level 3 in Health and Social Care Leadership or equivalent.
  • Experience managing supported living care services within a community setting.
  • Strong leadership skills and ability to foster positive relationships.
  • Excellent communication and IT skills.
  • Proven track record in managing complex care packages and supporting individuals with mental health conditions, personality disorders, autism, and learning disabilities.
