Job Overview

We are currently looking to recruit experienced Mental Health Practitioners to join our team in the North West.

The successful candidate will be expected to work on a rotational shift basis, including weekends and night shifts, to ensure effective care delivery.

Key Responsibilities
  1. Care Planning: Formulate, contribute to, and participate in the development and implementation of assessment and care planning systems in collaboration with the multidisciplinary team.
  2. Mental Health Assessments: Carry out generic and specialist assessments, including risk and provide therapeutic interventions/home treatments within the most appropriate setting in accordance with the service users and/or carers' needs.
  3. Risk Assessment: Assess whether users can be managed safely within the community promoting quicker recovery.
  4. Safety Plans: Assess Risks and collaboratively develop safety plans with users/carers.
  5. Liaison: Ensure effective liaison with inpatient units across the Trust to facilitate early discharges or to undertake reviews of those users placed out of area.
  6. Integrated Care Pathways: Assist in the development and implementation of integrated care pathways, including appropriate service user assessments/referrals and models of care.
Requirements
  • Registered Mental Health Nurse or Dip.COT/SROT
  • Evidence of post registration study / continuing professional development e.g. ENB 998 / Mentorship in Practice
  • PSI Training
  • Approved training in phlebotomy
Competencies
  • Able to communicate effectively both verbally and non-verbally
  • Possess excellent listening and negotiation skills
  • Ability to work as a member of a multidisciplinary team
  • Ability to lead the process of standard setting and audit

The salary for this role is £40,258 - £50,149 per annum pro rata, dependent on experience.
