Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our team at Swaffham & Downham Primary Care Network. As a Clinical Pharmacist, you will play a key role in providing expert medication advice and support to our patients and healthcare professionals.

Main Responsibilities

  • Conduct clinical medication reviews to optimise patient care and safety
  • Provide expert advice on medication-related issues to GPs and other healthcare professionals
  • Develop and implement medication safety protocols to reduce harm and improve patient outcomes
  • Collaborate with the multidisciplinary team to improve patient care and safety
  • Stay up-to-date with the latest medication guidelines and best practices

Person Specification

  • Degree in pharmacy, MPharm/BPharm GPHC registered
  • Independent prescriber qualification
  • Minimum of two years post-graduate experience in secondary, primary, or community care
