Job Summary

We are seeking a highly motivated and forward-thinking Clinical Pharmacist to join our team at Swaffham & Downham Primary Care Network. As a key member of our pharmacy team, you will play a vital role in delivering high-quality patient care and innovative service development.

Main Responsibilities
  • Conduct clinical medication and long-term condition reviews to address medication-related issues and optimize medication.
  • Make recommendations to GPs and other healthcare professionals for the management of long-term conditions, prescribing issues, and referrals.
  • Ensure patients are referred to the appropriate healthcare professional.
  • Advise patients on minor ailments and long-term conditions.
  • Ensure medicines are safe, appropriate, and effective for patients.
  • Undertake medicine reconciliation and process administrative letters.
  • Provide clinical expertise on medication-related issues to the multidisciplinary team.
  • Undertake regular prescribing-related audits and quality improvement projects.
  • Action medication safety alerts.
  • Ensure medicines are prescribed according to national and local guidance and the local prescribing formulary.

Job Requirements
  • Degree in pharmacy, MPharm/BPharm, GPHC registered.
  • Independent prescriber qualification.
  • Minimum of two years post-graduate experience in secondary, primary, or community care.
