Community Care Specialist

Lancashire County Council is seeking a dedicated Community Care Specialist to join our team.

Job Summary:
This permanent position involves working as part of a dynamic team to deliver high-quality community care services. As a Community Care Specialist, you will be responsible for assessing and meeting the needs of service users, ensuring they receive the best possible support to maintain their independence and well-being.
About the Role:
You will work closely with service users, their families, and other professionals to identify and address their needs. This will involve conducting assessments, developing care plans, and coordinating services to ensure seamless delivery.
As a key member of our team, you will contribute to the development and implementation of new initiatives and programs aimed at improving community care outcomes.
Requirements- Registered Social Worker with Social Work England (SWE)
- Ability to work flexibly, including weekends, to meet the needs of service users
- Excellent communication and interpersonal skills
- Strong assessment and planning skills
- Experience of working in community care or a related field
