Job Title: District Nurse

We are seeking a skilled and dedicated District Nurse to join our community nursing team at Newport Pagnell Medical Centre. As a District Nurse, you will play a vital role in providing high-quality nursing care to patients in their own homes, working closely with our multidisciplinary team to deliver holistic and evidence-based care.

Main Responsibilities:
  • Provide skilled nursing interventions to patients of all ages and conditions, working in partnership with the community nurse team and general practitioners.
  • Undertake delegated duties under the supervision of the community nurse team leader or senior on duty.
  • Maintain close liaison with the community nurse team leader, the team, GP, and other members of the primary care team and wider health and social care team.
  • Apply current infection control policies and universal precautions.
  • Encourage patients towards self-care and independence.
  • Maintain up-to-date knowledge of methods of rehabilitation.
  • Demonstrate, teach, and supervise the nursing care that families/carers can safely undertake.
  • Ensure that adequate medical supplies, equipment, and other materials are maintained.

We are a large training practice based in the town of Newport Pagnell with another surgery in the Willen area. Our team is dedicated to delivering high-quality care to our patients, and we are looking for a skilled and dedicated District Nurse to join our team.

Person Specification:
  • Current NMC registration with an active PIN.
  • Previous experience working as a practice or community nurse.
  • Experience of working in a primary care environment.
  • Experience of chronic disease management, infection control, rehabilitation methods.
  • Assisting with quality initiatives, i.e. benchmarking.
