About the Role

We are seeking a skilled Community Psychiatric Nurse to join our team at NHS Professionals. As a Community Psychiatric Nurse, you will play a vital role in providing high-quality care to patients with dementia and their families.

Key Responsibilities:

  • Conduct comprehensive assessments of patients with dementia to identify their needs and develop personalized care plans.
  • Work collaboratively with multidisciplinary teams to ensure seamless care coordination and delivery.
  • Provide education and support to patients and their families to promote self-management and independence.
  • Develop and maintain strong relationships with community healthcare providers to ensure effective care pathways.
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences.

Requirements:

  • Minimum of 6 months paid experience as a Community Psychiatric Nurse in a UK hospital environment within the last 3 years.
  • Compliance with NMC Professional Registration and adherence to current nursing practices.
  • Full UK driving license and own vehicle.
