Job Summary:
We are seeking an experienced Diabetes Specialist Nurse to join our team at Connaught Resourcing. As a key member of our primary care team, you will be responsible for managing diabetes patients in community settings, including GP practices, care homes, and health centres. Your expertise will be invaluable in identifying sub-optimally controlled patients and optimising their oral therapy.
Key Responsibilities:
* Conduct clinics in community settings and provide education, support, and advice to patients with Type 2 Diabetes
* Identify sub-optimally controlled patients and optimise their oral therapy
* Work in a pro-active manner to refer patients for specialist insulin treatment where necessary
* Exercise corporate nursing responsibilities
Requirements:
* Diabetes diploma or equivalent
* Minimum of 3 years experience managing diabetes in primary care
* Knowledgeable and confident in providing education, support, and advice to patients with Type 2 Diabetes
* Skills and knowledge in optimising oral hypoglycaemics
* Full driving licence
* Current NMC registration
* IT literacy, including primary care systems
What We Offer:
* Competitive annual salary of £45,000 + package
* Annual bonus incentive scheme (worth up to 10% of annual salary)
* Car allowance or company car (depending on contract) + fuel card for business miles

* Daily lunch allowance when in practice
* 25 days annual leave, plus bank holidays (increasing after 5 years service) - pro rata for part time
