Job Summary

Quarriers is seeking a skilled Transition Mentor to join our team. As a Transition Mentor, you will play a vital role in supporting young people to develop their independent living skills and achieve their goals.

About the Role

This is a full-time permanent position working 39 hours per week. The successful candidate will be responsible for providing one-to-one support to young people, helping them to develop their life skills and achieve their goals.

Key Responsibilities

  • Provide individualized support to young people, helping them to develop their independent living skills
  • Work with young people to set and achieve their goals, and provide ongoing support and guidance
  • Develop and maintain positive relationships with young people, their families, and other stakeholders
  • Contribute to the development and implementation of individualized support plans
  • Participate in regular team meetings and contribute to the development of the team

Requirements

  • Reliability, flexibility, and a positive attitude
  • Ability to build trusting relationships with young people and their families
  • Empathy and excellent communication skills
  • Excellent team player
  • Ability to apply transferable life experience and skills to support and enable individuals
  • Essential to have a driving license
