**Key Responsibilities**

  • Provide person-centered support to individuals with disabilities, promoting their independence and well-being.
  • Develop and maintain positive relationships with service users, their families, and other professionals.
  • Contribute to the development and implementation of support plans, ensuring they are tailored to meet the individual needs of service users.
  • Participate in ongoing training and professional development to enhance your skills and knowledge in disability support.
  • Collaborate with colleagues to ensure a seamless and effective delivery of support services.
