We are a friendly and supportive team at Minster Medical Practice, seeking an experienced Practice Nurse to join our nursing team.
Job Summary
We are looking for a candidate with a passion for delivering high-quality patient care to work with us at our surgery. The successful candidate will have experience in chronic disease management and be able to provide a holistic nursing service to all patients attending the surgery.
Main Responsibilities
- Assist with chronic disease management
- Provide a range of services to agreed practice guidelines/protocols
- Assess patients' physical, psychological, and social needs
- Deliver health promotion activities to the practice population
- Manage minor trauma, acute, and chronic wounds
- Help maintain disease registers in line with local and national guidelines
- Provide lifestyle advice and screening to aid in the prevention of possible future illnesses
About the Role
This is a varied role that will cover all aspects of practice nursing, including chronic disease management. The successful candidate will be able to work independently and as part of a team to deliver high-quality patient care.

- Previous general practice nursing experience
- NMC Registration
- IT literate
