About the Role
We are seeking an experienced Practice Nurse to join our friendly and supportive team at Minster Medical Practice. As a Practice Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our practice.
Key Responsibilities
- Assist with chronic disease management and provide a holistic nursing service to all patients attending the surgery.
- Deliver a range of services to agreed practice guidelines/protocols, including health promotion activities and minor trauma management.
- Assess patients' physical, psychological, and social needs and provide lifestyle advice and screening to aid in the prevention of possible future illnesses.
- Manage disease registers in line with local and national guidelines and maintain accurate records.
- Collaborate with senior nursing staff and GPs to ensure seamless patient care and referrals.
Person Specification
We are looking for a candidate with:
- Previous general practice nursing experience.
- NMC Registration and IT literacy.
- Desirable: experience of chronic disease management and diabetes or asthma/COPD diploma.
