Nurse Practitioner

We are seeking a highly skilled and dedicated A&E General Nurse to join our team at Sandwell and West Birmingham NHS Trust. As a key member of our healthcare team, you will be responsible for providing high-quality patient care in our community hospital setting.
Main Responsibilities:- Participate in clinical supervision and contribute to the ongoing assessment of patients to determine their individual needs and requirements.
- Assist in the development and implementation of care plans, ensuring that patients receive the best possible care and support.
- Work collaboratively with multidisciplinary teams to deliver Intermediate Care and provide medically optimized patients with discharge planning and safe discharges post an acute stay in hospital.
- Provide education and support to patients and their families, promoting self-care, safety, and good health practices.
- Maintain active NMC registration and adhere to NMC professional guidelines and registration.
- Demonstrate a consistent high standard of care in infection control and prevention.
