Job Overview

The Community Mental Health Team is seeking a skilled and motivated Mental Health Nurse to work within an established integrated, multi-disciplinary team, focusing on individuals with severe and enduring mental health problems.

Main Duties
  1. To be responsible for the care coordination of a designated caseload of clients with severe and enduring mental health problems.
  2. To ensure that the Care and Treatment Plans are adhered to and that standards are met in line with the Mental Health Measure (Wales).
  3. To act as a point of contact for GPs and other interface services to offer advice, support, and signposting to appropriate agencies.
  4. To formulate and develop standard and complex outcome-focused care and treatment plans.
  5. To ensure that a comprehensive risk assessment is completed and documented for actual and potential risk.
  6. To be responsible for supporting clinical duties such as assisting with the running of the depot/clozapine clinic and the administration and supervision of long-acting injections.
  7. To be responsible for assessing individuals in crisis, completing comprehensive assessments of an individual's health and social care needs, and agreeing the most appropriate course of action.
